BCHC SERVICE PLAN AND MODEL OF CARE 2008 - 2015 Finalised in early 2008, the Service Plan identifies community health needs for Ballarat and surrounds, and future development directions for Ballarat Community Health Centre. The Service Plan provides a framework for the organisation to institute change from the base of existing services, if appropriate, in order to achieve agreed objectives, or projected changes in services to clients, service structure and profile.
The Service Plan is designed to guide sustainable future organisational development in order to respond to community needs, and relevant government policy directions.
