Clinical Services - Early Intervention in Chronic Disease Management
This program offers a range of services that can assist you in managing your long term health conditions. The aim of the program is to provide self management support so you can become actively involved in your healthcare.
Who’s eligible:
People who have been diagnosed with one or more of the following conditions:
- Diabetes (Type 1 & 2)
- Chronic heart conditions (CVD, heart failure)
- Chronic lung conditions (COPD, asthma)
- Musculoskeletal conditions (arthritis, chronic pain)
- Depression and anxiety associated with chronic disease
- Long term health conditions that could benefit from lifestyle interventions
What can the program do for you?
Assist in:
- Understanding your health condition/s
- Developing and following a treatment plan
- Monitoring and responding to changes in your symptoms
- Managing the physical, emotional and social impact on your condition
- Adopting lifestyle practices that reduce risks and promote health
- Understanding and having the confidence to use health and support services appropriately
What does the program involve?
A key worker is assigned to work with you to identify your goals and assist you by providing a range of services, group programs and support networks.
An assessment appointment will be arranged.
An individualised wellness plan will be established by you and the key worker, in consultation with your GP, to support you in managing your condition.
This might include:
- Health Coaching – one to one self management support
- Education Programs
- Physical Activity Groups
- Self Management Courses
- Referral to allied health services such as physiotherapy, dietetics, diabetes education, counselling services, podiatry, etc
- Regular communication with your GP
There is no cost to join this program; however some of the services and programs may attract a small fee.
