Clinical Services - Doctors Clinic
Ballarat Community Health's General Practice Clinic provides patients with a full range of services with our General Practitioners and Practice Nurse including examinations, treatment advice and referrals. This friendly, non judgmental
clinic is part of a multi-disciplinary team of both internal and external providers within Ballarat and District.
How to make an appointment
Appointments are essential for both the Sebastopol and Smythesdale Clinics and can be arranged by phoning 03 5338 4585.
Clinic sessions are limited and therefore advance notice should be given
where possible if appointments
are no longer required.
Cost
Concession and non-concession card holders welcome. Patients with a concession card will be
bulk-billed. Non–concession card holders will be asked to pay their accounts on the day of consultation, Cash and EFTPOS payments are welcome. Some procedures will incur minimal costs.
(Medicare cards must be presented at every appointment).

Transport and Parking
Parking is situated at both ends of the complex and directly across the street in Vickers Street.
Alternatively, catch the Delacombe/Sebastopol Number 19 bus stopping at the front door, or Ballarat Number 12 Davis bus and dismount at the corner of Albert and Vickers Streets, there is a short walk of 500 meters west from that corner.
