The three Ballarat Community Health Doctors Clinics provide a full of general practice medical services. Our General Practitioners and Practice Nurses are committed to improving medical care for patients in regional and rural communities in Ballarat and the surrounding district.
The Clinics adopt a friendly, non-judgemental approach in the delivery of examinations, treatment advice and referrals. Committed to both health promotion and prevention our clinics utilise a range of internal and external providers to deliver comprehensive care.

How to Make an Appointment
Appointments are essential for all of the clinics and may be arranged by calling (03) 5338 4585. Clinic sessions are limited and therefore advance notice should be given where possible if appointments are no longer required. Alteration or cancellation of appointments must be made 24 hours before appointment. All missed appointments will be recorded.
After Hours and Emergency Services
If you require urgent after hours medical attention please ring 000 or present to your nearest Emergency Department at Ballarat Health Services or St John of God. For after hours medical advice please call (03) 5338 4585:
- You will be transferred to WestVic After Hours Advice or you may call them directly on 1800 641 395.
- In the event you are not transferred directly to WestVic After Hours Advice you will hear a recorded message explaining what to do.
- Please advise WestVic After Hours Advice that you are a registered patient of the Ballarat Community Health GP Clinic.
- A trained nurse will help you to assess your needs and provide advice.
- Further assessment may be needed in which case the nurse will contact the doctor on duty at the Ballarat Health Services Emergency Department to arrange this.
- The Ballarat Community Health GP Clinic will be informed of any advice and treatment that you have received but all health records will remain confidential.
Our clinics welcome both public and private patients (including concession card holders);
- Patients with a current concession card will be bulk-billed
- Non-concession card holders will be asked to pay their accounts on the day of consultation. Cash and EFTPOS payments are welcomed
- Some procedures/services may attract a small fee. These fees will be discussed before they are incurred
Patients remain responsible for payment of any fees from outside providers to whom they may be referred. Costs will be confirmed and provided to patients in writing when making the appointment/referral.

Parking and Transport
Parking is available at both ends of the complex as well as in a separate car park immediately opposite the main entrance in Vickers Street.
Public transport options include the No. 19 Delacombe/Sebastopol bus which stops immediately outside the main entrance. Alternatively, the No. 12 Ballarat bus can be taken. Please dismount at the stop at the corner of Albert and Vickers Streets to proceed to the centre.
Parking is available at the front of the building in the Learmonth Road Service Road along the full length of the centre.
The Wendouree West Bus stops in Gillies Street and then it is a quick walk through the DSE car park to the GP Clinic.
Parking is available at the front and side of The Well where the clinic is housed.
The Golden Plains Shire Shopping Bus will stop at the clinic upon request.
Staff will ask a set of questions to ensure every effort is made to allocate the appropriate length of time for your appointment. Allowance for urgent or longer appointments will be made in all consulting sessions. Please indicate at the time of booking if you are aware that you will need a longer appointment.
Telephone Access to Doctors
Doctors in the clinics may be contacted during normal surgery hours. If the Doctor is with a patient, a message will be taken and the reception staff will advise you when it is likely that the Doctor will return your call. Your call will always be put through to the doctor in an emergency.
Results and Reminders
Following referral for tests your Doctor will advise you when results will be available. Please call the clinic to obtain results. If you test result requires follow up by the Doctor, our Customer Service staff will contact you as soon as possible to arrange a follow up appointment. Reminders can be set on request to remind you of consultations. A recall service is also available to remind you of preventative health checks/screening appointments and/or care plan reviews. Please ask staff about these services anytime.
Managing Your Health Information
Patients of our practice have the right to access their personal health information under the provisions of the Health Records Act 2001. On request for access to personal health information, our practice documents each request and endeavours to assist patients in granting access where possible and in accordance with the legislation. Staff will forward the patient request to the patient’s doctor to check for exemptions. Exemptions to access must be noted and each patient or legally nominated representative must have their identification checked prior to access being granted. A full copy of our policy is available from each reception.
We Value Your Feedback
We invite patients to provide verbal or written feedback at all times.
Feedback forms are located in the waiting rooms and reception areas of our practice. Information used from the feedback (which can be anonymous) will be used for future planning of practice facilities or services. Written acknowledgement of feedback will be sent upon request.
Complaints may be made to the Practice Manager or to Ms Robyn Reeves, CEO, Ballarat Community Health.
